About this event

On December 6, 2023, we will honor CHP’s three decades of commitment to our vision, mission, and values, all centered in the Civil Rights movements of the 1960s, when health care was declared not just a human right, but a civil right for ALL. More than 50 years later, we continue to be tasked with defending this declaration. That is one reason why CHP was incorporated in 1993.

Today, we are an association of ten community health care clinics that collectively serve communities from Daly City to Gilroy. Our membership includes federally qualified health centers, school health clinics, free clinics, and family planning centers. We advocate for systems that provide quality care for all in a manner that is humane, affordable, and sensitive to their circumstances. We also advocate for local, state, and federal policies that institutionalize health care as a fundamental right.

Community Health Partnership (CHP) represents ten community health centers serving 41 sites in Santa Clara and San Mateo Counties, providing them with resources and expertise to deliver high quality, affordable care to our diverse community. CHP gives its members a collective voice to reach and educate policy makers, funders and community leaders – supporting local health centers’ efforts to shape health policy, secure funds, and strengthen the health care safety net. CHP also collaborates with members to drive best practices for quality care, maximize resources, improve technology tools, and navigate the changing health care environment.
